Health department retains programs

San Juan Basin Health Department’s Prevention and Population Health Division announces the continuation of these services during the next state fiscal year (July 1, 2015 to June 30, 2016):

Money has been received to employ a nurse to work with the Temporary Assistance for Needy Families Program at The Department of Human Services.

TANF assists parents in ensuring children are immunized, assesses the need for family planning services, ensures children in the program are assessed for developmental delays, coordinates any necessary referrals and helps parents set health goals. The health department received $86,619 for this program.

The Nurse Family Partnership is funded by the Colorado Department of Human Services and has provided services to Archuleta, La Plata and San Juan counties for more than 13 years. The partnership is a nurse home visitation program for low-income, first-time mothers. The health department received $471,390 for this program.

The Single Entry Point Program is funded by the Department of Health Care Policy and Financing. The program establishes home- and community-based services within Archuleta, La Plata and San Juan counties with the goal of preventing nursing-home placements for the elderly and disabled. The health department received $554,000 for this program.
