Health coalition to promote advance care planning

Mercy Regional Medical Center is one of the partners in the Community Health Action Coalition’s effort promote advance care planning.

The Community Health Action Coalition has been awarded $199,927 in funding from the Colorado Health Foundation to promote advance care planning in La Plata County over the span of two years.

The coalition’s new advanced care planning initiative, “My Life: Deciding in Advance,” will increase the number of adults actively engaged in the decision-making process about their health.

The initiative will educate people about how to consider their wishes regarding health care if they are unable to speak or communicate them to loved ones; train professionals to competently discuss and facilitate completion of advance directives to document those wishes; and strengthen community-clinic connections to assure directives are more readily accessible in the health care system.

The coalition is made up of multiple partners, including Mercy Regional Medical Center, Axis Health System, La Plata County Senior Services, Four Corners Health Care Center, La Plata Medical Society, Colorado Regional Health Information Organization, Southwest Colorado Bar Association and others.

The two-year grant funding began in October and will conclude September 2018.
