To streamline payments and provide continuity of services throughout the COVID-19 pandemic, the city of Farmington is requesting vendors apply to receive payments via ACH.
The ACH authorization form may be accessed online at www.fmtn.org/978/ACH-Authorization-Form.
The completed form can be returned via fax at (505) 599-1553 or mailed to: City of Farmington Finance, 800 Municipal Drive, Farmington, NM 87401.
