The La Plata County Public Health Department’s main office service window at 185 Suttle St., Ste. 100 will be open from 12:15 to 4:15 p.m. Nov. 17 to 20.
Birth and death certificates can be ordered directly through the Colorado Department of Public Health and Environment at cdphe.colorado.gov/vitalrecords.
Residents can submit applications and make payments online.
Private well water samples may be dropped off at 185 Suttle St. and payments can be made using the posted QR code. Frequently requested applications and well water test kits are also available in the lobby.
