For the past several years, the following partner organizations have conducted health coverage outreach and enrollment projects that have provided application assistance and health literacy education to thousands of individuals and families: Archuleta County Department of Human Services, Axis Health System, La Plata County Department of Human Services, La Plata Family Centers Coalition, Mercy Regional Medical Center, San Juan Basin Public Health and area health insurance brokers.
The ongoing mission of this partnership is to collaboratively pool resources and efforts to provide outreach, education and health insurance enrollment assistance to those who are uninsured or need assistance maintaining their health coverage in Archuleta and La Plata counties.
Reminder: enrollment in Health First Colorado (Medicaid) is open year-round, so those who remain uninsured are encouraged to visit any of the collaborative partners to check for enrollment eligibility.
