Community Health Action Coalition suggests area residents think about advance care planning during the holiday season and complete advance directive documents for loved ones.
CHAC has an initiative, funded by Colorado Health Foundation, to educate La Plata County residents ages 18 and older about health care, advance care planning and advance directive documents.
