Community Health Action Coalition will offer free educational presentations to groups that want to learn about advance care planning.
The initiative, “My Life: Planning in Advance,” is funded by the Colorado Health Foundation to educate La Plata County community members age 18 and older and strives to stimulate conversations about health care plans and advance directive documents.
Four advance directive documents and a conversation guide can be downloaded at www.chaclaplata.org.
