Behavioral health survey available

The Citizens Health Advisory Council is asking residents to complete a survey about the state of behavioral health in La Plata County.

Behavioral health is described – for the purpose of this project – as mental health and responsible substance use.

Using survey results, the council will create a community behavioral health plan.

To take the survey, visit www.chaclaplata.org/survey.asp. The survey takes about 15 minutes to complete.

Adult role model nominations sought

San Juan Basin Health Department, supporting the Celebrating Healthy Communities Coalition, wants to hear from young people about who they see as positive adult role models in La Plata County.

People younger than 21 can nominate an adult who exemplifies qualities such as modeling responsible, healthy behavior or positively affecting the lives of young people.

The adult could be a parent, family member, community elder, teacher, coach, neighbor, group leader or other adult.

Every time someone submits a nomination, his or her name is entered to win a Kindle Fire HD, donated by Boys & Girls Club of La Plata County.

Nomination forms can be found at www.sjbhd.org/celebrating-healthy-communities.

Nominations are due by Saturday, and award winners will be notified in April.
