San Juan Regional Medical Center announces COVID-19 to home clinic

San Juan Regional Medical Center will expand its care for COVID-19 patients outside of the hospital.

The COVID-19 to Home (C2H) Clinic is now available for patients who do not have a primary care provider, in an effort to coordinate care and provide close monitoring to eligible patients who are either discharged home for self-isolation and/or not ready for hospitalization.

The C2H protocol is based on Vanderbilt University Medical Center’s Care Model. Every patient tested within our system who receives results that are positive, suspected positive or negative but the patient may still have COVID-19-like illness will receive a call to set up an appointment with a C2H providers. Those providers will conduct routine telemedicine visits to follow up with the patients to ensure safe and effective care at home until they have recovered.

Patients who are interested in this program may call (505) 609-6284 to be screened for enrollment. Providers who would like to refer a patient and/or get a copy of the protocol are also asked to call this number.
