San Juan Regional Medical Center has been recognized for exemplary performance in health care transformation by the National Rural Accountable Care Consortium. The hospital is among the top 15% of enrolled practices to achieve this designation.
San Juan Health Partners Internal Medicine and Specialty Services, a division of San Juan Regional Medical Center, began working with NRACC in 2015 to receive support in managing population health and improving patient outcomes through the Transforming Clinical Practice Initiative.
They conducted 178 annual wellness visits in 2018 and 242 in 2019. Between the annual wellness visits and chronic care management appointments, they have increased patient participation in preventive initiatives appropriate for their health needs.
Rates now exceed national averages in several measures:
Influenza immunizations: 49.8%, compared with the national average of 39.3%.Pneumonia vaccinations: 80%, compared with the national average of 46.3%.Medication documentation: 98.8%, compared with the national average of 89.3%.
