Care Coordination Central will offer its biannual dialogue, “The Future of Care Coordination,” from 11:30 a.m. to 1:30 p.m. Oct. 17 in the conference room at La Plata Electric Association, 45 Stewart St.
The organization’s goal is to facilitate communication and collaboration among health providers to improve quality of care, satisfaction and cost outcomes for community members in Southwest Colorado.
